Request for Proposal: Cancer Care Community Grants

Program Details:
The Southwest Georgia Cancer Coalition is seeking to fund up to seven Cancer Care Community Grants to benefit cancer patients and their families who reside within our 33 county service region. (Refer to www.swgacancer.orghttp://www.swgacancer.org>www.swgacancer.org> for a complete listing) These grants are provided by proceeds raised from the 2007 RiverFront Run sponsored by the Coalition. These grants have 3 primary goals:

    - To improve the ability of cancer patients and their families to manage their disease
      effectively.
    - To assist organizations in the timely delivery of cancer treatment and support.
    - To promote cancer education and support for cancer patients and their families from
      diagnosis through end of life care.

Organizational Background:
The Southwest Georgia Cancer Coalition is a collaborative network of four regional cancer centers, public health and academic institutions, community service organizations, faith-based groups and cancer survivors committed to reducing cancer mortality in Southwest Georgia. The Southwest Georgia Cancer Coalition is dedicated to advancing the understanding of cancer in our region, while mobilizing communities and strengthening networks to provide complete access to high quality care for all individuals. The Cancer Coalition receives partial financial support from the four regional cancer centers and incalculable in-kind support from community, business, academic and other health care partners.

Eligibility and Selection Criteria
Nonprofit community-based health or advocacy organizations that directly support cancer patients or their families within our 33 county service area will be considered and are encouraged to apply. The Cancer Coalition is especially interested in programs that reach the patient and/or family directly and provide services that may include but are not limited to educational, psychosocial, pharmaceutical, or nutritional support.

Total Award:
Up to seven recipients will be awarded between $2,000 and $5,000 each to facilitate their program goals.

Project Summaries should not exceed three pages (10 font or greater) and should describe:

    - The target population, the health issue facing this population and contributing factors.
    - Components and activities of this project and the desired results. Please include an
      organizational background, project timeline and budget.
    - The applicant organization's ability to address the problem and the organization's relationship       to the target population. Please include current staffing and appointed leadership for the       project.
    - Projected sustainability of the project after completion of grant funding.
